Why the Handoffs Are Easy to Get Backwards

Underwriting and pricing split at the top by market: group, individual exchange, and Medicare Advantage each run on different risk-pooling logic and regulatory timing, so there's no single "underwrite a policy" flow that applies across all three. Claims adjudication forks again by claim type, with pharmacy claims often routing through a separate PBM relationship entirely rather than the medical claims engine, which most generic outputs collapse into one box. Provider network contracting and credentialing also have to complete as their own long-cycle flow before a claim can adjudicate against that provider at all, a precedence that's easy to state and easy to get backwards. And utilization management sits as a gate between care delivery and claims payment that can loop a claim back for review, so the real flow has a feedback path a straight-line diagram won't show.
