AI Governance in Practice: Moving Beyond Policies to Enforcement

There is a version of AI governance that exists in most large organizations. It takes the form of an AI use policy—a document, usually well-written, that defines acceptable and unacceptable uses of AI, establishes principles for responsible AI deployment, and articulates the organization's commitments on fairness, transparency, and accountability.
And then there is what actually governs how AI is built and used in the organization, day to day. In most organizations, these two things are only loosely connected.
The gap between documented AI governance and operational AI governance is where most enterprise AI risk accumulates. It is where shadow AI proliferates. It is where poorly governed AI systems get deployed without review. It is where performance degradation goes undetected. And it is where, when something goes wrong, the organization discovers that its policies existed as aspiration rather than enforcement.
This article is about closing that gap—not by writing better policies, but by building governance into how AI systems are built, deployed, and operated. That is what Module 5 of The CIO's AI Playbook is concerned with: not governance as a compliance exercise, but governance as an operational capability.
Why Policies Alone Are Not Governance
AI use policies matter. They establish organizational intent, provide a basis for training and communication, and create legal documentation of the organization's standards. But they have structural limitations as governance mechanisms:
They depend on individual compliance. Policies work when individuals know about them, understand them, and choose to follow them. Each of these conditions can and does fail. Developers under delivery pressure take shortcuts. Business users deploy AI tools without notifying IT. Vendors embed AI capabilities into products the organization uses without disclosure.
They cannot govern shadow AI. The most significant governance gap in most organizations is not the AI systems that went through a review process and then violated policy. It is the AI systems that never went through a review process at all—the Microsoft Copilot subscription a department bought on a corporate card, the third-party SaaS tool with embedded AI that changed its terms of service, the internal automation someone built using an AI API and shared across the team.
They cannot detect degradation over time. AI systems that pass initial governance review can degrade over time—as data distributions shift, as model providers make updates, as usage patterns diverge from the original design assumptions. Policies create a point-in-time standard; they provide no mechanism for detecting when deployed systems fall out of compliance with that standard.
They cannot substitute for technical controls. A policy that says "all AI outputs affecting customer decisions must be reviewed by a human" is not enforceable through the policy document. It is enforceable through technical controls in the AI system that route certain output types to a human review queue. The policy states the intent; the architecture enforces it.

The Five Components of Operational AI Governance
Governance that actually works in production enterprise environments consists of five interconnected components:
Component 1: AI System Inventory and Classification
You cannot govern what you cannot see. The first requirement for operational AI governance is a comprehensive, continuously updated inventory of AI systems in use across the organization—not just the systems IT built, but the SaaS products with embedded AI, the departmental AI tools, the AI components embedded in vendor-managed software.
Building this inventory requires three activities:
Active discovery: Scanning the technology stack for AI-enabled tools, requiring disclosure from vendors as part of procurement review, and establishing a reporting requirement for departments deploying or purchasing AI-enabled tools. No discovery mechanism is complete—shadow AI will always exist at the margins—but active discovery dramatically reduces the invisible portion.
Risk classification: Each identified AI system should be classified by risk level based on the consequences of AI failure, the degree of human oversight in the decision, and the reversibility of AI-influenced outcomes. A three-tier classification is typically sufficient:

Lifecycle tracking: The inventory must reflect not just what AI systems are deployed today, but what state they are in—active, under review, deprecated. AI systems that are no longer actively maintained but are still running represent a governance risk that is systematically undertracked.
Component 2: Technical Controls Embedded in Architecture
Governance requirements that depend on individual compliance fail. Governance requirements enforced by system architecture succeed—because they cannot be bypassed without deliberate effort that would itself be visible.
Technical governance controls in enterprise AI include:
Input guardrails: Filters that prevent certain types of inputs from reaching AI models—PII that shouldn't be processed by a given system, content categories outside the system's authorized scope, inputs that are likely to be adversarial.
Output validation: Validation logic that checks AI outputs against defined standards before they are delivered—format validation, confidence thresholds, content policy compliance, required element presence—and routes below-standard outputs to review or fallback handling.
Access controls: Authentication and authorization that determines which users can access which AI capabilities, with logging of all access. AI systems that can surface information from across organizational data need access controls as rigorous as any other system with that capability.
Human-in-the-loop routing: Logic that automatically routes AI outputs to human review based on defined criteria—output confidence below threshold, decision category requiring mandatory review, user role without authority to act on AI recommendation without approval.
Audit logging: Systematic capture of model inputs, outputs, user interactions, and system state for every AI interaction. The log schema should be defined as a governance requirement before system deployment, not added as an afterthought.

Component 3: Pre-Deployment Review Process
Tier 1 and Tier 2 AI systems require a formal review process before production deployment. The review process validates that the system meets governance standards before it begins affecting real decisions.
A functional pre-deployment review examines:
Technical validation: Does the system perform at acceptable accuracy, reliability, and latency under realistic production conditions? Has it been tested on data representative of what it will encounter in production, including edge cases and adversarial inputs?
Governance control validation: Are all required technical controls in place and functioning? Has the audit logging been verified to capture the required information? Is human-in-the-loop routing tested and confirmed?
Data governance: Has the data used for training, fine-tuning, and inference retrieval been validated for use permission, quality, and lineage? Are privacy and compliance requirements addressed?
Impact assessment: What is the population of people or decisions affected by this AI system? What are the potential failure modes and their consequences? Has a bias and fairness assessment been conducted where applicable?
Operational readiness: Is there a defined monitoring plan, performance baseline, alerting configuration, and incident response procedure in place before go-live?
The output of the review process should be a formal deployment decision—approve, approve with conditions, or reject—with documented rationale. This documentation becomes the audit trail for the deployment decision.
Component 4: Continuous Monitoring and Audit
AI governance is not a one-time gate at deployment—it is an ongoing operational responsibility. AI systems that were well-behaved at launch can degrade over time, and the monitoring infrastructure must detect this before it causes harm.
Performance monitoring tracks whether the AI system continues to meet the accuracy, reliability, and latency standards established at deployment. Performance baselines established before deployment provide the benchmark. Deviations from baseline trigger investigation and, where warranted, remediation or decommissioning.
Drift detection specifically monitors for distribution shift—changes in the data the AI system encounters that move it away from the distribution it was validated on. Concept drift (the relationship between inputs and outputs changes) and data drift (the distribution of inputs changes) are distinct phenomena requiring different detection approaches.
Fairness and bias monitoring for Tier 1 systems tracks whether AI outcomes vary systematically across demographic groups or other protected characteristics. This is not a one-time assessment at deployment—it requires ongoing monitoring as usage patterns and populations evolve.
Anomaly detection monitors for unexpected AI behavior—outputs that fall outside expected distributions, interaction patterns that suggest adversarial use, access patterns that suggest unauthorized use.
Audit sampling for high-risk systems involves periodic human review of a sample of AI decisions and the data that informed them. Automated monitoring catches systematic problems; audit sampling catches problems that are subtle enough to evade automated detection.
Component 5: Governance Operating Model
Technical controls and monitoring are necessary but not sufficient. Governance requires human accountability—defined roles responsible for specific governance activities, clear escalation paths when issues arise, and regular governance review cadences.
The governance operating model defines:
Who owns each AI system: A named individual or team accountable for the AI system's performance, compliance, and ongoing governance.
Who reviews and approves: The composition and decision authority of the pre-deployment review committee—typically cross-functional, including technology, legal, risk, and relevant business functions.
Who monitors: The team responsible for ongoing monitoring—typically a combination of the system owner (for operational performance) and a central AI governance function (for compliance and standards).
How issues escalate: When automated monitoring detects an anomaly, or a user reports an AI failure, what is the escalation path? Who is notified? Who has authority to suspend a system pending investigation?
How governance evolves: AI governance standards must evolve as the technology, the regulatory environment, and the organization's AI use cases evolve. Who owns the governance standards, and what is the review cadence?
Governing Shadow AI: The Practical Challenge
No governance framework reaches every AI deployment in an organization. Shadow AI—AI tools used by departments or individuals without IT or governance awareness—is a persistent reality in every large enterprise.
The response to shadow AI should be pragmatic, not punitive. A governance program that responds to shadow AI primarily through prohibition and enforcement tends to drive AI use further underground rather than reducing it. A more effective response:
Reduce the friction of formal governance for low-risk use cases. If getting an AI tool into Tier 3 (low-risk) status requires a six-week review process, departments will avoid the process. Tier 3 review should be a lightweight registration, not a full assessment.
Provide sanctioned alternatives. For the most common shadow AI use cases—writing assistants, summarization tools, internal chatbots—provide a centrally managed, properly governed alternative. Shadow AI proliferates when legitimate alternatives are unavailable.
Create amnesty pathways. Departments that are already using AI tools should be able to bring them into the governance framework without punitive consequence. Amnesty periods that allow existing shadow AI to be registered, assessed, and either approved or retired dramatically improve inventory visibility.
Monitor at the network and procurement level. DNS monitoring, network proxy logs, and procurement review (corporate card purchases, SaaS subscription reviews) can surface AI tool usage that individuals haven't reported. This is a detective control, not a preventive one, but it helps maintain visibility.
Regulatory Landscape: What CIOs Need to Track
The regulatory environment for enterprise AI is evolving rapidly. While comprehensive regulatory guidance is beyond the scope of this article, several frameworks are directly relevant to enterprise AI governance:
EU AI Act (effective August 2024, phased enforcement through 2027): Introduces risk-based AI regulation across the EU. High-risk AI systems—defined by use case categories including employment, credit, and critical infrastructure—face mandatory conformity assessments, human oversight requirements, transparency obligations, and registration in an EU database. Organizations operating in the EU should map their AI system inventory against the Act's risk classification.
NIST AI Risk Management Framework (AI RMF): A voluntary US framework that provides structured guidance for identifying, assessing, and managing AI risk. The AI RMF has been adopted as a reference framework by many regulated US industries and is increasingly referenced in sector-specific guidance from financial regulators, healthcare regulators, and others.
SEC guidance on AI in financial services: The SEC has issued guidance and proposed rules addressing AI use in investment advice, trading, and financial reporting. Financial services organizations using AI in these contexts face specific disclosure, fairness, and oversight requirements.
Sector-specific guidance: Healthcare (FDA guidance on AI-enabled medical devices, ONC interoperability requirements), financial services (OCC, CFPB, FFIEC guidance), defense (DoD AI ethics principles and contractor requirements), and other regulated sectors each have specific AI governance considerations that overlay the general frameworks.
